Micromechanical fatigue damage of human bone can be quantitatively assessed non invasively by ultrasonic NRUS techniques applied on cortical bone (cortical and trabecular elements). Different correlations between fatigue damage characteristics and nonlinear ultrasonic parameters were shown for cortical versus trabecular regions and for VOIs located at various sites in the investigated specimens. The microdamage density in VOI1 located on the non-damaged metaphyseal zone was in the same range as the microdamage density found on the endocortical lamella in the femoral diaphysis of obese subjects with comparable BMI values in a previous study of the same group of subjects [59], confirming previous findings by Marotti et al. (2007) [56]. Increased fatigue damage from non-damaged to damaged regions was observed in VOI1 (mean rank difference=17.62 m) and VOI2 (mean rank difference=8.91 m). No significant difference in damage characteristics was found between trabecular and cortical regions in VOI1 for both the averaged and SD values of microcrack density. In VOI2, microcracks density was significantly higher in trabecular compared to cortical regions. The number of microcracks was on average twice higher in trabecular than in cortical bone. A strong correlation was found between the fatigue damage characteristics measured in VOI1 by NRUS with those measured in cortical (Cr.Le r=0.71) and trabecular (Cr.Le r=0.80) regions by microCT in a study from our group, as well as with microCT data obtained in a previous study [37]. The damage density measured with NRUS in VOI1 showed a large variability (i.e., from 3.8 m-2 to 18.18 m-2). In VOI2, the lower variability (i.e., 0.01 #/mm2-0.16 #/mm2) was found in cortical regions, while an intermediate variability was found in trabecular regions (i.e., from 2.34 m-2 to 10.72 m-2). The observed strong correlations between the damage density and nonlinear ultrasonic parameters confirmed the sensitivity of the technique to early phase of microdamage accumulation as described in previous studies [14]-, [16], [75] and [37].

Finally, the relation between nonlinear elasticity and damage characteristics was also determined. The nonlinear elastic parameter f highly correlated with the density of microcracks measured in the crack region, suggesting that nonlinear elasticity increases in bone with microdamage accumulation. This study shows that nonlinear elasticity is a useful tool to quantitatively monitor microdamage accumulation in bone. Thus, nonlinear elasticity can be used to relate the newly determined “damage density to its elastic nonlinearity” to assess bone remodeling. Whereas, assessment of damage accumulation by NRUS appears promising to reveal a break-down mechanism of the bone matrix, this technique cannot provide further information on the location of microcracks in the bone, once the crack is broken. Finally the experimental design, high damage dose used, the young age of the specimens and the spatial resolution [6] are the main limitations of this study.
