a 61-year-old man who had sustained a fall from a motorcycle fell on his right knee with a weight of 300kg, causing his right tibia to fracture through the lateral cortex. the patient experienced severe pain and difficulty in walking at the time of admission. upon physical examination, the patient exhibited obvious signs of swelling in the affected knee and a clear line of fracture at the lateral tibial cortex (fig. 6 ). axial ct scans revealed that the lateral tibial plateau was not in the original position and that a small piece of bone was present in the tarsal tunnel (fig. 7 ). 59 surgery was performed, with a posteromedial incision used to expose the lateral tibial plateau. the fractured fibula was then displaced laterally to provide a greater field of view for reduction. the main articular surface of the tibia was located on the lateral side of the fracture, and the interosseous membrane was found to be ruptured. the lateral tibial plateau was reduced, and a 3.5-mm reconstruction plate was used to fix the two articular fragments. then, an anterolateral approach was used, with adjustments made as needed according to the specific location of the die-punch fragment. after c-arm x-ray imaging confirmed satisfactory reduction of the fractures and the articular surface was flattened, the main supporting bone fracture plates were placed on the medial or anterolateral side of the distal tibia along the original displacement direction of the fractures. miniplates or screws were used as needed to assist in fixation.
